New Employee Information

We are glad you have made the choice to join the Mines community. This page will provide you with links to the required new hire forms that will allow us to complete the hiring process. Forms may be printed and completed prior to your first visit to Human Resources. For information on how to access campus computer resources, click here.

FORMS REQUIRED FOR ALL FACULTY, STAFF AND STUDENT EMPLOYMENT

  • Original Social Security Card: Mines is required by state and IRS guidelines to pay employees by the name that is printed on the original security card. If you do not have a card or need a replacement, please contact the Social Security Administration for information. Mines policy allows us to pay one month with out obtaining a copy of your social security card. A copy of receipt of application will serve as proof that a card has been requested and will allow HR to enter your information for payroll.
  • I-9 ImmigrationCOMPLETE SECTION 1 ONLY - You must bring original documents to HR to complete the form. Please provide proper documentation as stated on the I-9 form. Employees who can not provide proof of eligibility for work in the US within 72 hours of employment at Mines will not be allowed to work until proof of eligibility is established.
  • PERA FormEnrollment into the Public Employees Retirement Association is mandatory for all employees. Participation in PERA is in lieu of paying FICA taxes, with the exception of Medicare tax. For more information on PERA, visit www.copera.org.   NOTE:  Students do not qualify for PERA - do not complete this form if you are a student employee.
  • Form SSA-1945The Social Security Act of 2004 requires state and local employers to disclose the possible impact of provisions on employees hired into jobs not covered by Social Security on or after January 1, 2005. For more information visit: www.socialsecurity.gov.
  • W-4 FormAlternately, complete the W-4 Information for Non-Resident Aliens.
  • Direct Deposit FormState statute requires that all employees are on direct deposit. Employees must provide a voided (or copy of) a voided check to ensure that account and routing numbers are correct. A deposit slip is not acceptable as the routing numbers are often different.
  • Gender & Ethnicity Self -DisclosureInformation provided on this form is not kept with the employee personnel file.
  • Individuals with Disabilities/Covered Veterans Self-DisclosureInformation provided on this form is not kept with the employee personnel file.

  • Travel Accident Insurance Form  NOTE:  Students do not qualify for Travel Accident Insurance - do not complete this form if you are a student employee.

FORMS FOR NON-RESIDENT ALIENS

INTERNATIONAL STUDENTS ONLY

In addition to the above documents, international students on visas must also provide the following documentation.

  • Work Authorization Letter from the CSM International Office
  • I-94:  Arrival/Departure Record
  • I-20 or DS-2019:  Certificate of Eligibility for Nonimmigrant (F-1) Student Status - for Academic and Language Students (I-20), Certificate of Eligibility for Exchange Visitor (J-1) Status
  • Visa/Passport
